I hereby agree to the following:

1. That I am participating voluntarily in a Class, Course or Workshop offered by CELESTIAL DANCE/DANI BRUNS. I recognize that pole dance fitness, aerial yoga, aerial fitness, aerial arts, yoga and personal training requires physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation and I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program.

2. I understand that I will not receive physical assists or adjustments to enhance or correct my body posture during class by the teacher unless she deems it crucial so safety. This is due to the current COVID precautions. I will take accountability for alerting the teacher and assistant of any injury or impairment in advance before class begins.

3. I consent to the collection and use of my personal images by photography or video recording. I acknowledge these may be used on the CELESTIAL DANCE/DANI BRUNS website and other social media such as Twitter, Instagram and Facebook, in newsletters and publications.

4. I knowingly, voluntarily and expressly waive any claim I may have against CELESTIAL DANCE/DANI BRUNS and staff or associates for injury or damages that I may sustain as a result of participating in the program.

5. I, my heirs or legal representatives forever release waive, discharge and covenant not to sue for any injury or accident caused by CELESTIAL DANCE/DANI BRUNS negligence or other acts.

6. I understand that an area for personal belongings is available during class, however, I agree that CELESTIALDANCE/DANI BRUNS or associates is in no way responsible for the loss or damage of any of my belongings while I attend class.

7. I agree to abide by the rules and regulations of CELESTIAL DANCE/DANI BRUNS as well as the venues and acknowledge that these rules and regulations may change from time to time.

8. All applicants under the age of 18 years must have consent, in written form, from a parent or guardian. To register or purchase from the CELESTIAL DANCE/DANI BRUNS online booking system, you must be over 18 years of age. You must ensure that the details provided by you on registration or at any time are correct and complete.

I have read the above release and waiver of liability and fully understand its contents.
